Check mandatory settings

Receive System E-mails : It should be enabled for the Super Administrator who need the approval process notification. For the settings go to User Manager at back-end and open Super User account.
Set activation link setting as explained below:

In Joomla!1.5, Go to Global Configuration -> System -> Users Settings
- Allow User Registration is set to YES
- New User Account Activation is set to YES
In Joomla!1.7, Go to User Manager->Options (shown at top right corner)

Allow User Registration is set to YES
New User Account Activation is set to Self
